I don't know about the rest, but the capsule is a genuine Neumann/Berlin M7 PVC whose backplate, judging from the bare wire which is soldered to its rim, seems to be grounded.

That would mean, the front diaphragm is hooked up to a de-coupling capacitor to filter out the DC of  the polarization voltage before its signal is fed into the grid of the tube. Not the cleanest of signal paths.

Does this capsule look like it might be able to be cleaned up to good condition?

Also, the backside seems to not be in as good of shape. Thoughts? Can a capsule with one poor side still be used in cardioid mode just fine? What do cleanup costs for capsules like this run, roughly?

Now that you show the other side of the story: Ouch! Game over.

Once one diaphragm of a dual-diaphragm capsule is penetrated, the acoustic damping is off, and the capsule is finished.

In addition, in your case there is probably already a short from the torn conductive diaphragm surface contacting the backplate.
