'white noise' problems with AD-16x

I just hooked up an AD-16X. I'm getting a white-noise type hiss coming from channels 6 and 10. If I mess with the power plug, turn it on and off a few times, it eventually goes away. I have done the reset, but no matter how I might get it working again, when I power everything down, then up again the problem is back. Sometimes it's only Ch6, sometimes only Ch10...

(The AD-16x is feeding an RME card via lightpipe into Nuendo, Mac G5, then via lightpipe into a DM24)

I have tried -
taking out the inputs to the AD-16x, makes no difference.
swapping out the lightpipes, not the problem.
switching word clock, from BNC to lightpipe, no difference.
switching inputs inside Nuendo, it follows that the outs of the AD16x are where the noise is coming from.

Anyone know if this could possibly be any kind of clock issue, or if it's a defect in the convertor?

Sounds like a defective unit to me.

I'd contact Apogee tech support.

Have you tried to connect it directly to the DM24 (running the Apogee on internal clock)?  I have experienced (not very often though) that some lightpipe interfaces are not the best of friends.  Hope you find a solution.

I tried the AD16x out to the DM24, both clocking from the AD16x and with the Dm24 set to internal clock. Same problem both ways.

I guess it's just defective. Trying to exhaust all options before Monday AM...
