Would anyone know where I can get a replacement board for this 709 opamp?

PCB or the chip itself? If the PCB is intact, you have an easy solution. The 709 14 pin package follows the standard dip 8 single package layout in the center pins. All you need to do is fit a machined socket in the correct position and then you can use a standard 8 pin dip single package opamp. Then you have mondo choices to select from, all of them light years beyond the ol' 709. I've done this in old Auditronic stuff and it worked out very well.
